The Audit-the-Auditor Strategy 🕵️♂️ 100% inline inspection is the standard, but in flexible medical device manufacturing material creep, seal variability and a host of other variables mean that batch-level QC is still heavily relied upon. I recently saw a brilliant human-factor strategy in a QC lab: Quality technicians were assigned specific factory zones to sample. Every hour, the shift leader rotated the staff to a different zone. Why this is an Inspection Masterclass: Eliminating Expectation Bias: If a tech looks at the same process all day, they subconsciously expect it to pass. Fresh eyes every 60 minutes break that cycle. The Internal Audit: By rotating, the second technician effectively audits the Pass criteria of the first. Traceability: It creates a multi-layered check on manufacturing of flexible components where seal all variables are assessed in a strong matrix format. The Lesson: High-end production equipment is only as good as the sampling strategy behind it. If your QC process doesn't account for all variables then its not doing its job right. A bad batch doesn’t start in the lab. It shows up in the lab but it usually starts somewhere much earlier in the process. Over time I’ve learned that most quality issues are already “decided” before QC even gets a sample. When results come out off-spec; high FFA, elevated peroxide value, or moisture issues the instinct is often to look at the lab first. But in reality, the real questions are usually upstream: What happened during storage of raw materials? Were process conditions (temperature, time, or flow) properly controlled? Was there any delay or interruption in production? Was the system stable from the beginning of the run? The lab doesn’t create defects, it only reveals them. And that mindset changes everything. Instead of just asking “what is the result?”, you start asking “what caused this result?” In many cases, quality issues are not sudden failures. They are gradual build-ups, small deviations in process conditions that compound over time until they finally show up in the final analysis. That’s why real quality control is not just about detection… it’s about understanding the process well enough to prevent problems before they happen. The more time I spend in this field, the more I realize that the lab is only the mirror. The real work happens in production. Five questions I ask before I will agree that a quality system is audit-ready. 1. If I pulled three CAPAs at random from the last twelve months, would the effectiveness verification still hold today? 2. If I walked the floor with the most recent training record, would the operators describe the procedure the same way the document does? 3. If a notified body asked for change history on a critical supplier, could I produce it inside an hour? 4. If a regulator asked the QC lab for the last calibration record on a critical gauge, would the answer come from a system, not a person? 5. If the head of operations was unavailable, would the quality team still know what production is doing tomorrow morning? A system that fails on any of these is not audit-ready. It is audit-lucky.

Because it means our quality system caught a potential error before it ever reached a patient. 🛡️ The "In-Date" Mystery 🔍 Interestingly, the reagent in question hadn't expired yet. This reminded us all that quality goes beyond just checking a date on a bottle. We had to dive deeper into: Open Vial Stability: How long has the bottle been breathing? Environmental Factors: Did temperature or light play a role? The "Invisible" Shift: Detecting subtle trends before they become major problems. Integrity in Action We didn't just "rerun and hope." We stopped, investigated the root cause, applied the Westgard Rules, and corrected the issue. In our lab, we don't release results until we are 100% certain of their precision. Patient safety isn't just a policy; it’s every action we take when the control chart goes out of bounds.
