Healthcare is evolving rapidly, but one challenge I’ve consistently observed in nursing is the difficulty in shifting long standing mindsets. As a young nurse, it can be frustrating to see policies and practices that clearly need improvement, yet the common response from senior colleagues is, “This is how we’ve always done it.” While experience is invaluable, holding too tightly to tradition can slow progress and limit the quality of care we provide. We are in a new era of healthcare one driven by evidence based practice, innovation, and patient-centered care. To move forward, we must be willing to question outdated systems and embrace necessary change. Some examples of nursing policies and practices that need re-evaluation include: 1• Rigid hierarchical communication structures that discourage junior nurses from speaking up, even in critical situations affecting patient safety. 2• Manual and paper-based documentation systems instead of adopting efficient digital health records. 3• Limited involvement of nurses in policy-making and leadership decisions, despite being at the frontline of care. 4• Inadequate staffing policies that do not reflect current patient acuity and workload realities. Progress in healthcare requires courage the courage to speak up, to challenge norms respectfully, and to advocate for better systems. This National Nurses Week, I want to recognize the true power behind patient care — our Nurse Practitioners. While patients often remember the diagnosis, treatment, or prescription, what they may not always see is the countless hours Nurse Practitioners spend coordinating care, documenting charts, handling compliance, following up on authorizations, and managing an overwhelming administrative workload on top of clinical responsibilities. You are not just treating patients — you are holding together the operational heartbeat of healthcare. “When Respect is Missing: A Nurse’s Perspective” Not every wound we see is physical. Some are felt… quietly, deeply, and repeatedly. As nurses, we are trained to care, to stay calm, to give our best—even in the toughest situations. But what happens when respect is missing? When your efforts are overlooked. When your voice is unheard in critical moments. When you’re treated as “just a nurse” instead of a skilled professional. It doesn’t just hurt—it affects how we work, how we feel, and how we grow. Because respect in healthcare isn’t a luxury. It’s a necessity. A respectful environment: ✔ Improves patient care ✔ Builds stronger teamwork ✔ Boosts confidence and decision-making And most importantly—it reminds nurses that they matter too. Seeing this post hit core! No break, extended time on feet, just about to break … then wait! There’s another call light another ASAP demand, sugar dropping, hanger setting in stomach growls, about to finally get personal hydration, suddenly see lunch trays rolling out and 7 pre-meal BS✔️REQUIRED! It’s not that your inexperienced or lack time management skill; when everyday top priority is pt safety when swallow may be compromised while roomie left on the toilet yelling for cna, then you still have the remaining 5 BS ✔️🤷♀️👩🏼⚕️😳🧞♀️that also sit right up top shelf priority as the one in front and the other in your peripheral crying out for assistance. Mind goes into instant safety first mode but suddenly pt #1 (a/ox2) grabs their full med cup of pills,Parkinson’s shaking them all over the sheets (unintentionally), while one large one lands in the patients mouth after diet was just changed to purree, the mind shifts again to pt #1 while you call out to assistance not able to easily leave….. all while you have pt#1 who is in danger of airway compromised, then pt#2 toileting at risk of falling without required assistance, and 5 remaining BS✔️now falling into 🤦🏼♀️potential risk of hypo/hyperglycemic. I LOVE my assistance on the floor who clearly recognize scope of license and each others personal obligation to the patient centered care. WITHOUT THE TEAM … we just cannot do it & for some over~looked reason … it’s the one that’s standing before the patient that seems to take it from all angles. The patient, the family, the neighbors, then supervisory/Admin staff who hears it from the above, if it’s a weekday then it’s therapy staff, the rounding NP or physician who looks to the one who remains on the floor with the keys “what was pt B blood sugar prior to lunch”, Dr asks … 🤦🏼♀️🤷♀️the one who’s taking it not only to her empty gut, her swollen feet/now ankles and knees, & dry & thirsty…. She thinks of her own aging mom who’s not looked after in that moment, as well as her own grown son with childhood TBI & recent breakthru seizure lead to near drowning while surfing. Yes, that nurse whose holding it ALL and still standing hoping to keep it all together at least long enough to eventually be able to elevate her license from that LVN load of responsibility to a more respected BSN, but only if she can hold it together. How can she explain all those scenario of obstacles without sounding like non-compliant psych pt herself 🤦🏼♀️. 👆🏽this is only one description of the many different chaotic predicaments compromising pt safety/quality of care and simultaneously compremising “the one with the keys” clinical competence in skill, patient care load, prioritization and time management and being wrongfully ridiculed whether directly or behind the scenes. My god RESPECT needs to be at ALL levels of in house facility care team. DON has command to set a tone, should be equipped with discernment to see the potential hazards amongst the layers of staff, trickled over effecting patients & the lack of respect does NOT allow for a positive flow in the patient environments. This is where they stay, this is where we only work. How we connect the information that is endorsed or not will 💯 cause an effect. #bringbackshiftchangehuddles
