Yesterday, we had a case requiring nasal intubation. While this procedure might not be a daily task for general practitioners, I made sure my junior doctors were fully involved in the process. Beyond the technicality of 'how'—choosing the right ETT, the insertion technique, and the fixation—the real value lies in the 'why.' We dived deep into the clinical decision-making, ventilation-oxygenation strategies, and the fundamental differences in equipment. In medical education, exposure is key. Even if they don't perform this specific procedure in the future, understanding the nuances ensures they become more well-rounded and observant clinicians.

Today, I read an interesting article entitled “Medical School Grading — Is ‘Good Enough’ Good Enough?” by Alexander A. Iyer, David A. Hirsh, and Richard M. Schwartzstein, published in the New England Journal of Medicine (NEJM) in 2025. The article examines whether pass/fail grading in medical education is sufficient. While it improves student well-being, it may reduce motivation and obscure excellence. Traditional grading is biased and inconsistent. The authors argue for balanced, competency-based assessment systems that ensure fairness, rigor, and meaningful evaluation rather than relying solely on simplified or rank-based grading approaches. Why do engineers and doctors do not agree sometimes? In those cases, at the best, they agree to disagree. I have spent decades interacting with doctors and we do have discussions, and at times arguments. We engineers are trained based on the principle of cause-and-effect relationship. Doctors on the other hand, of course, learn based on cause-and-effect but also based on correlation. In medical science, a high correlation is enough to conclude on something. My request to both the communities is to be mindful of this. That's all.
