Stanford and Harvard just published the most thorough review of clinical AI evidence from 2025. The report, *The State of Clinical AI (2026)*, looked at what actually works when AI leaves controlled research settings and enters real clinics. Not the headlines. The evidence. The findings: → Diagnostic reasoning is mixed. AI performs well in studies but struggles with incomplete information and time pressure—the reality of everyday medicine. → Prediction at scale works. Systems trained on wearable vital signs predicted patient deterioration 8–24 hours before standard hospital alerts. → AI as teammate outperforms AI as replacement. Radiologists using AI assistance detected more breast cancers without false alarm increases. Doctors using AI made better treatment decisions than those using conventional tools alone. → Patient-facing chatbots are spreading fast. Few studies track whether they actually improve health outcomes. Over 1,200 AI tools are already FDA-cleared.
