💭 Are we investigating safety… or designing it? 🎙️ Podcast review: Todd Conklin's Pre-Accident Investigations (PA Pod 596) – Incremental Safety Practices: Reductive vs. Inductive 📢 Listen: https://lnkd.in/dCc8MqFJ This episode explores Hollnagel's latest book focused on the strengths (and limits) of two complementary approaches to safety: · Safety I: reduce risk by fixing what goes wrong. · Safety II: build capacity so teams can adapt and keep care safe under pressure. Here’s a quick analogy that may help it click: Potholes on roads Safety I: fix the potholes. If the road surface is smooth, everything flows. The focus is on spotting defects and repairing them quickly: no pothole, no problem. Safety II: assume potholes will happen and design in resilience. Roads are used all day, every day and weather, heavy loads, and time mean deterioration is expected. So we build capacity (resilience) so small defects don’t become major incidents, for example: 1. Make the carriageway wider than the vehicle (room to adapt without catastrophic consequences). 2. Use suspension (absorbs bumps so the vehicle stays stable). 3. Use pneumatic tyres (deforms around small hazards and protects the wheel). In reality, we need both. Some hazards should be removed (repair the pothole).
