A common mistake in sleep technology is prematurely adding supplemental oxygen before optimizing CPAP titration. Instead of reaching for O2, consider adjusting IPAP without changing EPAP. This increases pressure support, which in turn can boost tidal volume. This adjustment can effectively address residual hypopneas, desaturations, and snoring. The hours of sleep are, in some ways, the most important training window for oral motor rehabilitation. During sleep, the conscious mind isn't compensating whatever position the tongue adopts is the default position, not the performed one. For patients with low resting tongue posture, mouth breathing, or compromised airway patency, those hours of unconscious default behavior are where the dysfunction compounds. The sleep-worn Oral Positioning Appliance protocol addresses exactly this window.
