The Common Roots Health Exchange, a Regional Action Network operating across four counties in the Piedmont region of North Carolina, is one of our 2026 Catalyst Awardees. This initiative brings together partners across four rural North Carolina counties to strengthen care, improve coordination of behavioral health and substance use services, and address social needs like food access. The action network will establish shared governance, data systems, and a closed-loop referral model to ensure more accountable, coordinated care for children, and families. The Southern Alliance for Public Health Leadership is a Regional Action Network operating across Georgia, North Carolina, South Carolina, and Tennessee. This multi-state network equips public health leaders with tools, data, and strategies to improve access to care, vaccination rates, and maternal and child health outcomes.
