We’re looking for a compassionate, organized, and proactive Care Coordinator to join the Population Health team at SIMEDHealth.
In this role, you’ll help patients successfully navigate their healthcare journey by coordinating care, supporting chronic disease management, encouraging preventive care, and assisting with transitions following hospital or facility discharge. If you enjoy helping people, solving problems, and working collaboratively with healthcare teams, this may be a great fit.
Key responsibilities include:
Coordinating follow-up care after hospital or facility discharge
Engaging patients regarding preventive screenings, chronic condition management, and recommended follow-up care
Serving as a liaison between patients, providers, insurance resources, and community services
Documenting patient interactions accurately and timely
Supporting quality improvement and population health initiatives
Collaborating with physicians, hospitals, home health agencies, and other healthcare partners
What we’re looking for:
High school diploma or equivalent required
Associate degree or health-related education preferred
3+ years of experience in a healthcare, physician practice, outpatient, or hospital setting preferred
Strong communication and organizational skills
Comfortable working independently in a remote environment
Proficiency with Microsoft Office applications
Ability to manage deadlines while maintaining professionalism and confidentiality
Ability to travel to clinics within the North Central Florida area (limited)
