Through consistent outreach—especially following hospital or emergency room visits—we ensure patients not only receive the care they need, but also feel supported in their recovery at home. Whether it’s helping schedule follow-ups, identifying gaps in care or connecting patients with resources like home health or meal services, the Patient Advocate Team works to create a seamless, full-circle care experience. This proactive approach allows us to intervene early and provide additional support when it matters most.   • You can’t help but SMILE at this! Brandon brings fun energy wherever he goes – and the strong connections he builds are reflected in how he shows up in the community beyond day-to-day work.

    What an incredible day last Friday, at the 2026 Spring Senior Expo in Davenport, Iowa! 🌸 I had the honor of volunteering for Anika H. with Senior Resource Partners as nearly 700 seniors came together to access free services — haircuts, health screenings, medication reviews, vaccines, and so much more. One of my favorite parts of the day? Being connected to programs like I-Smile Silver (Scott County Health Dept), which helps seniors find dental care, the Quad Cities Senior Olympics, therapy dogs, and countless other resources that make a real difference in people's lives. And yes — I may have spent part of my shift as "Pearl" the Tooth Mascot for I-Smile. 🦷😄 No regrets. Events like this are built on service and connection — and they are so vital to our local senior community. I'm deeply grateful to Oasis Health Partners® for fostering a culture of volunteerism and encouraging us to show up for the communities we serve.
