We have spent over two decades in primary care, long enough to watch it become a 27-hour-a-day job that leaves physicians burnt out and patients underserved. The math has been broken for a long time, and that's the problem we built Tom to address. In a recent Second Opinion Media interview with Christina Farr, David Carmouche, MD, our chief medical and commercial officer, sat down to talk about what it will actually take for AI transformation in primary care to deliver meaningful impact. I posed a thought experiment to David Carmouche, MD: What would it look like where a primary care physician had a patient panel of 5000, versus what exists today (1700-2000 roughly). With AI, could such a proposal even make sense? He outlined his thinking in this week's Second Opinion Media roundup. -- Passive monitoring for a large swatch of the patient to make time for the 10-12 patients who really need to get seen -- Patient models that allow PCPs to not be treated like the lower rung and increase their salary (perhaps a PEPM for the entire population with some bonus payments attached for outcomes?) -- Data interoperability -- let the PCP operate a "control pane" of sorts, with all the relevant information pouring in and any results flagged by AI This isn't science fiction; many of the models exist today and there's an argument to be made that the technology is already better than the status quo where so few patients have any access to primary care at all.
