Most LTC pharmacies dispense medications before the billing setup is complete. The payer hierarchy might be half-configured, the census file just landed, and the order entry team is already queuing up the next batch. So what do you do? In our recent Office Hours session, Derek Taylor, PharmD, Senior Director of Pharmacy Client Services at Infinx, shared a triage rule that works: If your order entry staff can tell whether a patient is Medicare A, Medicaid, or Part D, they can make a smarter dispensing decision. Dispense a 3-day supply instead of a full month. This gets the medication to the patient while limiting your exposure if the payer information turns out to be wrong. It also buys you enough time to confirm coverage before a full 30-day supply goes out the door. The pharmacies that handle incomplete information well have trained their front-end staff to ask the right questions and follow basic contingency rules. When information is missing, they have a plan. The ones that struggle are winging it every time.
