We’re honored to have been selected by the Centers for Medicare & Medicaid Services (CMS) to serve as the End Stage Renal Disease (ESRD) Network contractor for Network 15 (Arizona, Colorado, New Mexico, Nevada, Utah and Wyoming) and Network 18 (Southern California). Together, these networks support more than 76,000 dialysis patients across 845 dialysis and transplant centers. The ESRD Network Program has played a vital role in advancing the quality, safety and effectiveness of kidney care across the United States for nearly five decades. As the program continues evolving with an emphasis on advanced technology and a customer service approach, Comagine Health is proud to be part of the next chapter. Over the next five years, Comagine Health will focus on four transformative priorities: • Empowering patient choice through education about home-based dialysis options and transplant pathways • Strengthening integration and improving access so that patients receive coordinated, comprehensive care • Supporting whole-person care, including mental health and preventive services • Improving interoperability and data sharing to drive quality improvement We’ve partnered with CMS to serve the ESRD community since the program began in 1978, and we look forward to continuing this important work in the years ahead. Community Co-Design Is Key to a Successful Community Information Exchange Stories have always been at the core of human connection. Sometimes these are oral or written, and sometimes, stories are told with data. A Community Information Exchange (CIE) is a technology system that helps different entities share important information with each other. Within a CIE, these entities can safely share data — like what support a person needs or services they already receive — so the entity can connect a person to health-related social need (HRSN) services to live a healthy life. In the context of a CIE, stories are told through the health and social data of individuals needing and receiving services. Each piece of data represents a person’s lived experience, and honoring these stories means respecting the right of every individual to decide when, with whom and for what purpose their information is shared. This commitment is not only a legal requirement, it is a moral imperative that should guide every aspect of a CIE program.
