  • Care Synergy has named Dr. Nancy Hernandez as the organization’s new Vice President of Education, effective May 18. Dr. Hernandez is an experienced healthcare leader and educator with expertise in nursing leadership, clinical education, quality improvement, instructional design, and healthcare innovation. She has led system-wide initiatives focused on talent development, evidence-based practice, strategic partnerships, and operational excellence. Among her accomplishments, Dr. Hernandez helped develop and implement clinical AI tools that significantly improved sepsis outcomes while reducing hospital length of stay.   • Care Synergy is excited to announce that Vennita Jenkins has joined the organization as PACE President. A highly respected leader in senior services and healthcare operations, Vennita brings more than two decades of executive leadership experience focused on improving the lives of older adults through compassionate, community-centered care. Prior to joining Care Synergy, Vennita served as CEO of Senior Housing Options, where she led strategic initiatives that expanded affordable housing opportunities, strengthened partnerships with healthcare providers and government agencies, and improved operational performance across multiple communities. Her background also includes serving as Center Director for InnovAge Greater Colorado PACE, where she oversaw operations for one of the nation’s largest standalone PACE centers. Kerri returns to Care Synergy with extensive experience in hospice quality, compliance, education, and operational leadership. Over the course of her career, she has held leadership positions at The Elizabeth Hospice, The Denver Hospice, and previously served in multiple quality and education leadership roles within Care Synergy.   • Care Synergy’s Nursing Assistant Training Program continues to deliver outstanding outcomes for future Certified Nurse Aides (CNAs). Through a comprehensive 76-hour program that includes classroom instruction, simulation lab training, and hands-on clinical experience in a skilled nursing setting, students gain the knowledge and real-world skills needed to succeed in patient care. The results highlight the strength of the program: 100% pass rate on the written CNA licensing exam (national average: 70–80%) 82% first-time pass rate on the skills exam (national average: 63–76%) Beyond technical skills, the program emphasizes patient rights, mental health awareness, and meaningful interactions with patients and families—preparing students for compassionate, high-quality care.   • Care Synergy and the CAPABLE National Center are encouraging healthcare organizations to apply for a new Medicare innovation opportunity focused on helping older adults stay safe and independent at home. The Centers for Medicare & Medicaid Services recently introduced the LEAD Model (Long-Term Enhanced ACO Design)—a new initiative designed to strengthen coordinated care through value-based models. Within this program is RISE (Resilience and Independence in a Safe Environment), a falls-prevention initiative modeled on the evidence-based CAPABLE program. RISE supports older adults by: ✔ Reducing fall risk ✔ Improving the ability to perform daily activities ✔ Supporting independence at home ✔ Helping prevent costly hospitalizations Healthcare organizations forming or joining an Accountable Care Organization (ACO) can apply to participate in the LEAD Model. Selected participants will enter a 10-year agreement with CMS and help expand access to evidence-based falls prevention. Jason is an accomplished HR leader with extensive experience in healthcare operations, employee relations, and organizational development. Most recently, he served as Director of Human Resources at Optimal Home Health and Hospice, where he led initiatives that improved organizational culture and reduced employee turnover while supporting multi-state compliance and workforce operations. In his new role, he will partner with leaders across the organization, bringing a disciplined, hands-on approach to HR operations with an emphasis on employee relations, optimizing HRIS technology workflows and data insights, strengthening retention strategies, and supporting a high-performing HR team across the Care Synergy enterprise. Jason also brings a strong service background as a veteran of the United States Navy, reflecting the discipline, adaptability, and mission-focused leadership he brings to the role. Last week, Tricia Ford, Chief Operating Officer – Hospice/CAPABLE, hosted a breakfast conversation about the CAPABLE Program at the Jewish Association Serving the Aging Annual Conference 2026. The session brought together staff and partners from Jewish Association Serving the Aging to learn more about the CAPABLE model and explore the strong alignment between our organizations. Both share a commitment to helping older adults remain safe, independent, and empowered in their homes for as long as possible. Through meaningful discussion, participants explored how our shared philosophy—supporting functional independence through holistic, interdisciplinary care—can strengthen efforts to help older adults age in place.
