Today, the House Energy & Commerce Committee is holding a hearing examining the future of the Medicare Physician Fee Schedule and physician payment reform. The American College of Surgeons is actively engaged in these discussions and continuing to advocate for policies that protect patient access to surgical care and appropriately value the work surgeons provide every day. Surgeons are caring for increasingly older, sicker, and more medically complex patients while also facing growing administrative burdens, continued cuts to work RVUs, and excessive prior authorization requirements that can delay care. Proposals to reform the Medicare Physician Fee Schedule by shifting reimbursement from specialty to primary care are gaining traction. On face value this is very appealing but the evidence that this will improve health outcomes is limited. At the same time, under budget neutrality, these changes risk reducing access to lifesaving specialty care. Physician payments account for about 9% of US national health expenditures—arbitrary shifts in reimbursement of one specialty to another will have a negligible impact on healthcare affordability compared to the rapid growth in spending on hospital and pharmaceutical costs. Primary care physicians face an increasing administrative burden and uncompensated time. But the same is true for all physicians, irrespective of specialty. The path towards affordability can’t be simply boiled down to primary care versus specialty care. Patients need both.
